What's caused this plant to brake down?

Too much light (intensity and duration) for CO2 level. Most physical damage of this sort is lack of CO2 (or boisterous fish).

Must be to much light but no algea or hungry otto cats as no algea in tank.

This is a slow growing tank? Why I'm dosing salts, got good light and Co2

Can't find the missing fish must of broke down and sucked in the filters?
