Holes in leaves

Any idea what could be causing holes in the leaves of my amazon swords? EI dosing with trace, KNO3, KH2 PO4 also add K2SO4 a couple of times a week. Co2 injection, drop checker always light green. Good water turn over with 3 external filters and a Koralia for extra movement. 204 watts of lighting, the tank is approx 350l. Could I be missing somthing thats casing the holes, the tank does have ottos but never see them on the swords, all over plants are fine.

Hi,
Check first for predation. If it's not an attack by snail or other varmint then the next cause is poor CO2.

some reason why there is hole in leaves it because of insect.

yes some insect like grasshopper, worm etc love to it leaves.

maybe some reason are because of not healthy soil where the plant grows or there is

no enough sun or photosynthesis that regulates the food on the leaves.
 
Do you have Clown Loach in there ? From the Pic looks like an eating pattern
