Replant melting

i have lindernia rotundifolia on my tank from the first i setup about 2 month ago, and it growing well until this problem came up, about 3 weeks ago everytime i trim this lindernia, i always replant it again, the problem is the replant slowly always melting, it like fail to adapt, and it only happen to the replant, what cause it? need some help here

hi there, do you have some photos? I think that would be useful to see the conditions of your tank otherwise it would be difficult to help.
