java farn dieing off

hi to all members

i am having some problems growing java farn, i add ie mix as most do on this site that suit the size tank that i have there is lots of flow from filters and co2 added.

the problem with the farn is that it growws new plantlets on the old leaves wich is ok but then the plantlets die off at a fantastic rate and i cant seem to resolve the matter.

help
