Thanks for everyone's help. In the end I don't think it was white spot or hole in the head but instead a impact injury. I did a couple of anti fungus- bacterial treatments and it went away in 3 or 4 days. All good
I have an injured young keyhole that I am keeping close eye on and recently seen some spots on his face. No other fish has shown signs of white spot but as this fish is injured it might be weaker. Could it be something else as it doesn't really look like white spot to me.

Crazy. I have this tank with keyhole cichlids too! I never see anyone with them anymore. Great fish. I upgraded my lights but find the lid the tank comes with doesn't work with anything but t tubes. Had to create my own lid
